To our valued patients:
Due to a number of changes placed upon us by insurance carriers, we are forced to change our billing policies. A copy of your current insurance card and photo identification (such as a drivers license) will be required at your appointment.
We do participate in ALL MAJOR COMMERCIAL INSURANCE COMPANIES, as well as Medicare. We do accept children under 12 for the Medicaid program. Copays and coinsurances are expected to be paid at the time of service.
DEDUCTIBLES. Many of the commercial insurance plans have a high deductible causing a potential large balance on your account. In order to increase our efficiency, we will require a copy of your credit card at the time of check in to cover these high deductibles.
The credit card information will be held in a secure area until your balance is determined. It usually takes 2-3 weeks before we receive an insurance Explanation of Benefits (EOB). Once received, we will call you for permission to use your card. If you are not home we will leave a message. If we do not hear from you in two days, we will bill your credit card for the balance and send you a copy of the credit card receipt and the EOB.
Payment in this manner will be advantageous to you and to us. You will no longer have to mail in payment or call in your credit card information; for us, it will save us from sending out a bill.
REFERRALS. If a referral is required for your insurance plan, it is your responsibility to provide us with the referral prior to being seen by one of our physicians or PAs. Please note that your referral is specific for the provider you are seeing that day. If you are seeing a different provider, you need to obtain a corrected referral.
